Patient Forms

For Patients coming to Digestive Healthcare Endoscopy Suite at 2616 Sherwood Hall Lane, Suite 307, Alexandria, VA 22307

Please, print out and complete these forms at your convenience and bring them back with you to your appointment, along with your driving license (or official picture ID) and your insurance card .

Preparation for Colonoscopy

Please, carefully review and follow the instructions you have been given for your procedure. If you have any questions about whether your prep is working or not, or you feel you cannot complete the prep for whatever reason, please call us at 703 - 780 - 7010 and enter 1 to leave an emergency message for the doctor. The doctor will call your back and advise you on how to proceed further.

Insurance and Billing

Please contact our office to verify acceptance of your insurance plan. Qualifications for insurance coverage may differ due to the uniqueness of each procedure. We are happy to file insurance for your reimbursement as long as you are free to choose your own gastroenterologist.

What To Expect

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It will also help alleviate any unnecessary anxiety you may be feeling prior to your first appointment. Educate yourself on your symptoms by reviewing the content on this website.

We would appreciate new patients arriving 5 to 10 minutes prior to your appointment time to complete any necessary paperwork. A brief consultation will be performed prior to setting up a procedure.

Procedures: For procedures you will usually need to arrive an hour before your scheduled procedure time and you will need a responsible driver to take you home (per hospital policy). The hospital will not allow a taxi ride home. If you need the phone number of a responsible transportation service, please inquire.

Changing a Hospital Procedure Date: If you need to change your procedure date due to an emergency, please let us know as soon as possible.

From Beltway 95/495 (coming from either direction):

  • Take exit 177A South (Fort Belvoir).
  • Continue on Route 1 for approximately 5 miles.
  • Turn left onto Sherwood Hall Lane.
  • After going through the light on Sherwood Hall Lane you will make the second left before the sign that says 2616 Sherwood Hall Medical Building.
